Basically what happens is when you take a drug and it gets into the brain it changes the chemicals in the brain that are released. Specifically they usually work on two chemical; serotonin and dopamine. It is these two chemicals in the brain that effects the nerves in the body when you take drugs.

People design their drugs to specifically alter these two chemicals in order to get the effects that we all know about. Sadly this can be very dangerous. Because as you know the nerves run all over our body so by changing how they work it can be end badly.
